Visa Requirements

1. Original Passport with minimum validity of six months and two blank passport pages.
   2. Three recent colour photos with white background ( 3.5 cm x 4.5 cm ) .
   3. Duly filled two Application Forms ( Application for Crew Visa Form ) .
   4. Duly filled one Applicant’s Occupation Form
   5. One photocopy of passport information and applicant’s photo page.
   6. A photocopy of valid Crew License.
   7. Documents mentioning purpose of entry, Name and Address of concerned Company / Organizaiton.
   8. A photocopy of round-trip flight ticket.
   9. Applicants may be asked to provide other documents if and when necessary.

   10. Detail Reason for applying Multiple Entry Visa
   11. Applicants may be asked to provide other documents if and when necessary.
